Nurse Practitioner – NYS Veterans’ Home at Oxford
Location: Oxford, NY

Agency: New York State Department of Health (DOH)

Facility: New York State Veterans’ Home at Oxford

Classification: Salary Grade 24 (Competitive / NY HELPS)

Schedule: Full-time (Typically 40 hours/week, Monday–Friday, with on-call rotation as required)
Position Overview
The Nurse Practitioner (NP) at the NYS Veterans’ Home at Oxford serves as a key clinical leader within our 242-bed skilled nursing facility. Under the general direction of the Medical Director, the NP provides comprehensive primary, preventive, and specialized geriatric care to a dedicated population of veterans and their eligible family members. This role is essential in maintaining the highest standards of clinical excellence, ensuring our residents live with the dignity and quality of care their service deserves.

Key Responsibilities & Clinical Duties
Direct Patient Care & Assessment

  • Perform comprehensive physical examinations and health assessments upon admission and during routine intervals.

  • Diagnose and manage acute, chronic, and complex health conditions common in geriatric populations (e.g., dementia, cardiac disease, respiratory issues).

  • Order, perform, and interpret diagnostic tests, including laboratory work, X-rays, and EKGs.

  • Prescribe pharmacologic and non-pharmacologic interventions in accordance with New York State practice agreements and protocols.

Care Coordination & Interdisciplinary Leadership

  • Serve as a clinical lead in Interdisciplinary Team (IDT) meetings to develop and refine person-centered care plans.

  • Collaborate with RNs, LPNs, social workers, and rehabilitation therapists to ensure holistic resident support.

  • Provide clinical oversight for the prevention of skin breakdown, falls, and infections (Wound Rounds, Infection Control).

  • Assist in the "Patient Review Instrument" (PRI) and MDS assessment process to ensure accurate clinical documentation.

Communication & Advocacy

  • Act as a primary point of contact for families and residents regarding medical status, treatment changes, and end-of-life care goals.

  • Maintain detailed and timely electronic medical records (EMR) in compliance with state and federal regulations (CMS).

  • Educate residents and staff on health promotion, disease prevention, and self-management techniques.

Minimum Candidate Qualifications

  • Licensure: Current license and registration to practice as a Nurse Practitioner in New York State.

  • Certification: National certification as an Adult, Family, or Geriatric Nurse Practitioner (preferred).

  • Clinical Experience: Minimum of 3,500 hours of post-certification clinical experience (for independent practice status) or a valid collaborative agreement.

  • Legal: Must have an active DEA license for prescribing controlled substances.

  • Background: Must be eligible for New York State Civil Service appointment.

  • Interpersonal Skills: Exceptional verbal and written communication, with the ability to handle sensitive family discussions with empathy and professionalism.

  • Efficiency: Ability to manage a steady caseload in a fast-paced skilled nursing environment.

  • Technology: Proficiency with EMR systems and Microsoft Office (Word, Outlook, Teams).
