Job Description: Medical Director / Lead Physician (High Acuity Long-Term Care)
We are seeking a high-caliber Medical Doctor (MD) with a background in Urgent Care or Critical Care to spearhead a clinical revitalization project within our 30-bed high acuity Long-Term Care (LTC) unit. This role is designed for a leader who can effectively triage complex cases, reorganize clinical workflows, and restore premium patient care standards for residents with diverse chronic conditions. Role: Medical Director / Lead Physician
Setting: 30-bed High Acuity Unit (Long-Term Care)
Commitment: Up to 40 hours per week (Onsite)
Focus: Clinical Triage, Process Restoration, and Chronic Disease Management
Key Responsibilities
Clinical Leadership: Evaluate and stabilize patient care standards; implement protocols to improve outcomes in a high-acuity setting.
Triage & Management: Provide expert triage for residents with complex, multi-systemic chronic conditions.
Collaborative Care: Function as a core member of a multidisciplinary team (nursing, therapy, social work) to ensure holistic healthcare delivery.
Operational Oversight: Organize clinical priorities to reduce hospital readmissions and enhance the quality of life for long-term residents.
Qualifications
Education: Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO).
Experience: Proven experience in Urgent Care and/or Critical Care is required to manage the high-acuity nature of the unit.
Skills: Strong organizational leadership, the ability to thrive in fast-paced environments, and expertise in managing complex chronic conditions.
