Job Title: Licensed Practical Nurse (LPN) Location: NYS Veterans Home – Queens, NY Employment Type: Full-Time and Part-Time Positions Available Shifts: All three shifts (7 days a week) Resident Load: Provide care for a minimum of 20 and maximum of 40 Veterans Position Overview: Under the supervision of a Registered Nurse (RN), the Licensed Practical Nurse (LPN) provides direct nursing care and clinical support to residents in accordance with state regulations and facility standards. This role involves supervising Certified Nursing Assistants (CNAs), delivering hands-on care, and ensuring the health and well-being of Veterans in a long-term care setting. Key Responsibilities:Direct Patient Care
Provide personal care including bathing, dressing, grooming, and toileting assistance.
Assist residents with mobility, transfers, and ambulation using assistive devices.
Monitor residents’ physical and emotional well-being and report changes promptly.
Clinical Duties
Perform vital signs, weight checks, and assist CNAs with ADLs such as feeding.
Administer medications (oral, topical, injectable, inhaled) as prescribed and document accurately.
Provide wound care, including dressing changes and preventive skin care.
Collect specimens (urine, stool, sputum, blood via finger stick) for lab testing.
Monitor and report lab values and diagnostic results to RN or physician.
Insert, maintain, and monitor urinary catheters, IVs (as permitted), and feeding tubes.
Administer oxygen therapy and monitor residents on oxygen.
Communication & Documentation
Document all care provided and observations in the medical record promptly.
Communicate effectively with RN, providers, residents, and families within LPN scope.
Report changes in resident condition immediately to supervising RN or physician.
Participate in shift hand-off reports to ensure continuity of care.
Supervision & Delegation
Supervise and direct CNAs in daily resident care tasks.
Ensure CNAs follow care plans and report concerns.
Provide feedback, support, and guidance to CNAs.
Care Planning
Assist in reviewing and updating resident care plans under RN supervision.
Participate in interdisciplinary care conferences.
Implement interventions per care plans and physician orders.
Emergency Response
Recognize and respond promptly to medical emergencies (falls, seizures, cardiac/respiratory distress).
Notify RN immediately and assist in CPR and first aid as required.
Contact RN, physician, or emergency services when necessary.
Resident Advocacy & Support
Promote dignity, respect, and resident rights in all care activities.
Provide emotional support to residents and families.
Educate residents and families about health needs, medications, and care routines within LPN scope.
Qualifications:
Current New York State LPN license in good standing.
Ability to supervise CNAs and work collaboratively with RNs and interdisciplinary teams.
Strong clinical skills and knowledge of long-term care practices.
Excellent communication and documentation skills.
Must pass pre-employment health clearance, drug screening, and criminal background check.
