Title: Spiritual Care Coordinator
Position Type: Per Diem Client: New York City health system Location: Bronx, Brooklyn, Queens Hours: TBD Salary: $55 per hour
Our client seeks to be the premier provider of health services in the greater metropolitan area, ensuring access to health, supportive and community-based services across the continuum of need. Their range of health services include home care, hospice and palliative care for adults and children, rehabilitation and nursing care and the research-based institution for innovation in palliative care. They also offer health plans for Medicare and dual-eligible individuals. Their ability to serve at-risk members of the community is supported by the efforts of their non-for-profit foundation, which raises funds to help support their quality programs and services not reimbursed by insurance or government programs. In addition, the generosity of grateful families, private and corporate donors and grants, as well as their own employees, also support their specialized services.
Responsibilities:
Overall responsibility for coordination of pastoral support services to patients and families.
Provides direct pastoral services to patients and families.
Coordinates spiritual support with requested religious affiliations for patients and families.
Completes and prepares documentation from clinical contacts and patients.
Inputs clinical data into MISYS database. Maintains updated intake reports.
