Responsibilities:
In this role, you will assume the ongoing, primary responsibility to coordinate, implement and continually evaluate the home care needs of your patients.
You will also review patient insurance information; coordinate activities involved in each individual care plan and make sure that pertinent findings are shared with family and professional caregivers.
Your excellent communication, documentation and time management capabilities will be essential to your success. And all along the way, you will experience the respect for your opinions and the high level of autonomy you need to do the job right
Assess the patient for the appropriate level of care and utilization of agency resources.
Utilize motivational interviewing to engage patient/caregiver in the established plan of care.
Ensure the EMR accurately reflects: the current plan of care; the services provided; patient’s response to treatment; status of goal achievement.Â
Effectively communicate and coordinate the delivery of services across all disciplines, including physicians, PSM, WOCN, CDE, and MLTCP.
Accurately complete all OASIS data sets, within agency established guidelines, which reflect positive clinical outcomes and consistent utilization of industry established best practices.
Requirements
Graduate from an accredited School of Nursing; BSN preferred
Minimum of 1 year acute, Med-Surg Nursing experience – required
