Title: Registered Nurse Coordinator of Care, Home Care  Position Type: Permanent, Full-time
Agency: New York City health system
Location: Brooklyn, Queens, Manhattan
Hours: Monday – Friday, 9:00 am – 5:00 pm
Salary: $100K-119K plus $15,000 Sign-on Bonus or Student Loans Assistance, paid over the course of 3 years   Our client is the premier provider of health services in the greater metropolitan area, ensuring access to health, supportive and community-based services across the continuum of need. Their range of health services include home care, hospice and palliative care for adults and children, rehabilitation and nursing care and the research-based institution for innovation in palliative care. They also offer health plans for Medicare and dual-eligible individuals. Their ability to serve at-risk members of the community is supported by the efforts of their non-for-profit foundation, which raises funds to help support their quality programs and services not reimbursed by insurance or government programs. Responsibilities:
In this role, you will assume the ongoing, primary responsibility to coordinate, implement and continually evaluate the home care needs of your patients.
  • You will also review patient insurance information; coordinate activities involved in each individual care plan and make sure that pertinent findings are shared with family and professional caregivers.
  • Your excellent communication, documentation and time management capabilities will be essential to your success. And all along the way, you will experience the respect for your opinions and the high level of autonomy you need to do the job right
  • Assess the patient for the appropriate level of care and utilization of agency resources.
  • Utilize motivational interviewing to engage patient/caregiver in the established plan of care.
  • Ensure the EMR accurately reflects: the current plan of care; the services provided; patient’s response to treatment; status of goal achievement. 
  • Effectively communicate and coordinate the delivery of services across all disciplines, including physicians, PSM, WOCN, CDE, and MLTCP.
  • Accurately complete all OASIS data sets, within agency established guidelines, which reflect positive clinical outcomes and consistent utilization of industry established best practices.

Requirements
  • Graduate from an accredited School of Nursing; BSN preferred
  • Minimum of 1 year acute, Med-Surg Nursing experience – required
  • CHHA/LTHHCP experience preferred
  • Bilingual skills (Spanish, Cantonese or Mandarin)
