Title:  Registered Nurse (RN) - Hospice Case Manager  Position Type: Permanent, Full-time
Location: Manhattan, Brooklyn, Bronx,
Hours: 4 DAY WORK WEEK
Salary: $115K-119K plus $15,000 Sign-on Bonus or Student Loans Assistance, paid over the course of 3 years
Work Authorization: Must be authorized to work in the U.S. without sponsorship.

Our client seeks to be the premier provider of health services in the greater metropolitan area, ensuring access to health, supportive and community-based services across the continuum of need. Their range of health services include home care, hospice and palliative care for adults and children, rehabilitation and nursing care and the research-based institution for innovation in palliative care. They also offer health plans for Medicare and dual-eligible individuals. Their ability to serve at-risk members of the community is supported by the efforts of their non-for-profit foundation, which raises funds to help support their quality programs and services not reimbursed by insurance or government programs. In addition, the generosity of grateful families, private and corporate donors and grants, as well as their own employees, also support their specialized services.

Responsibilities:

In this field-based position, you will visit Hospice and Palliative Care patients to perform both physical and psychosocial assessments and electronically document visit occurrences. Patient visits will be made in both community and facility settings. As the Hospice Field Case Manager, you will develop and implement nursing plans of care and make recommendations for care based on patients' needs.

The schedule includes working one weekend per month and functioning in a backup On Call capacity 2-3 times per month. Car and escort service available for backup On Call shifts.

Requirements:
  • Associates/Diploma from an accredited school of Nursing; BSN preferred
  • At least two years’ experience in medical-surgical, community health, and/or oncology nursing preferred
  • Proficiency with Word and Excel preferred
  • New York State RN licensure and valid NYS driver's license required
  • Experience working within an interdisciplinary team environment
  • Effective communication, listening and teaching skills
  • Ability to input clinical data proficiently
