Title: On Call Center Triage Registered Nurse
Position Type: Permanent
Client: New York City health system
Location: Bronx, NY
Hours: 5 PM - 9 AM, 5PM - 1AM
Salary: $100K per year, plus $15,000 signing bonus

Our client, a large health care system in NYC, is seeking an Call Center Triage Registered Nurse. The On Call Center Triage RN is responsible for the emergent triaging of calls received from patients and/or caretakers to hospice and palliative pare and home vare for the purpose of telephonically assessing their needs in order to treat emergent situations that have developed during the On Call Center hours daily, 5 PM-9 AM. Provides patient physical and psychosocial health systems assessments telephonically, supporting the Plan of Care, “Goals of Care” based on the patient’s and caretaker’s needs/wishes, and in coordination with plan developed/placed by the primary RN Case Manager for the patient.

Responsibilities:
  • Reviews at the beginning of the shift all information via email to the HospiceCallCenter Inbox regarding changes and relevant information received.
  • Reviews the HospiceCallCenter Inbox continuously throughout shift while on duty.
  • Prioritizes the calls and visits to the patients based on information received.
  • Monitors NetSmart Task box hourly in order to dispatch on call visit staff and to ensure timely follow up and clinical coordination with the On Call Services Field RN.
  • Places “Tuck-In” call for telephonic follow up and may schedule an RN visit PRN when deemed necessary after triaged.
  • Records all telephone interactions in NetSmart “Clinical Notes” and/or “Tasks” as needed.
  • Escalates appropriate calls/situations to AOC as needed.
Requirements
  • Graduate from an accredited School of Nursing; BSN preferred.
  • A minimum of two years’ experience in either Hospice, Palliative, Critical or End-of-Life care preferred.
  • CHPN after one year of employment.
  • New York State RN license, CPR-BLS and valid NYS Driver's license.
  • Microsoft Office, Word and Excel.
  • Ability to be dependable and capable of using initiative and good judgment.
  • Possess good communication, listening, and organizational skills.
  • Possess strong clinical skills regarding end of life care
  • Ability to input clinical data proficiently.
  • Ability to effectively communicate in a customer service environment.
  • Ability to work flexible hours to meet the needs of the position.
  • Ability to coordinate the resources and personnel involved in a comprehensive interdisciplinary team environment.
  • Ability to work non-judgmentally with patient / family members of any culture, religion, socio-economic background or lifestyle.
  • Detail oriented and ability to set own priorities.
  • Ability to handle confidential aspects of assignments.
