Title: Licensed Practical Nurse (LPN) Position Type: Long-term, temporary (1+ years) Location: Bronx, NY Hours: Full-Time- 37.5 hours Salary: $36 per hour
Our client, an occupational therapy and rehab services organization, is seeking a licensed and experienced LPN to assess individuals applying for one of their transportation programs.
The LPN will work closely with applicants who may have mental health or cognitive impairments, ensuring that eligibility determinations are made based on their psychological and functional needs. The role involves conducting assessments, providing detailed reports, and offering recommendations to support the transportation needs of individuals with disabilities.
Responsbilities
Conduct assessments focused on mental health and cognitive functioning for individuals applying for the transportation program.
Evaluate applicants' functional abilities as they relate to their mental health and decision-making skills, ensuring their capacity for independent transportation.
Prepare detailed reports summarizing assessment findings, including diagnostic information and recommendations regarding eligibility.
Collaborate with a multidisciplinary team, including healthcare professionals and transportation coordinators, to ensure fair and accurate eligibility determinations.
Communicate assessment results and recommendations effectively to applicants, their families, and caregivers as necessary.
Stay updated on state and federal regulations regarding disability rights and access to transportation services to ensure compliance.
Maintain confidentiality and adhere to HIPAA standards while handling sensitive psychological and personal information.
Requirements
Must have at least 1 year experience as a LPN
Graduate from an accredited Licensed Practical Nurse Program
Drivers license and care required for all visits except in Manhattan
Strong office / computer skills; Phone systems, Microsoft Office Suite, Microsoft Teams, Outlook, EMRS
