Title: Access Center Registered Nurse Position Type: Permanent, Full-Time Client: New York City health system Location: Brooklyn and Manhattan Hours: TBD Salary: $100K per year, plus $15,000 signing bonus
Our client, a large health care system in NYC, is seeking an Access Center RN.
Responsibilities:
Reviews pre-admission documentation prior to admission appointment with patient.
Reviews documentation from referral sources for eligibility
Reviews payor source information for the extent of coverage and reimbursement.
Contacts patient and/or health surrogate to introduce self, verify appropriate health surrogate if not the patient, and confirm appointment time and location for admission
Reviews findings with Hospice Medical Director in order to develop nursing plan of care for patients’ goals of care within accordance of applicable laws, regulations, and agency policies.
Provide emotional support to patients and their caregivers.
Maintains compliance with dress code during patient and referral interactions.
Adapts communication content and style for maximum effectiveness with different audiences.
Establishes and promotes an ongoing collaborative relationship between the agency and/or other facilities, direct caregivers, physicians, nurses, discharge planners, case managers, social workers, etc. to ensure coordination of referrals.
Offers any new referral source a Hospice informational meeting.
Maintains required referral source credentialin
Requirements
Graduate from an accredited School of Nursing, BSN preferred.
A minimum of two years’ experience in either Hospice, Palliative, Critical or End-of-Life care preferred.
New York State RN license, CPR-BLS, and valid NYS Driver's.
National Board Certification in Hospice & Palliative Care (CHPN) preferred or obtains certification within one year of hire.
